There are two main referral pathways, depending on the service on which you require.
Panel referral – This consists of a group of service professionals including Social Services, the local authority, police, domestic violence co-ordinators and other social care professionals. The aim of this panel is to assess referrals and to ensure allocations are made to the relevant service and to clients with the greatest need. A referral through this pathway will need to be via one of the professional services listed above. This applies to our scheme based mental health, learning and physical disabilities properties.
Self referral – This type of referral is where an individual, or someone acting on their behalf (such as a probation officer), can contact the service directly and make a self referral. An initial telephone assessment is conducted before an interview is offered. This applies to both the women’s refuges and homeless hostel.
Both referral pathways will require each applicant to complete an application form prior to the interview, which is considered on its individual merit. If the application meets the criteria, then a Tenancy Support Co-ordinator will arrange a convenient time to meet in order to conduct a full needs and risk assessment. This is used to determine if the level of need is suitable for the service. We have a strong emphasis on equality and diversity and we will consider any application to our services from all areas.
