Please use the application form below to confirm if you would like a room to be re-decorated.  The room that you choose will not have been decorated within the last four years. 

The form will need to be returned no later than 30 January 2009 either via your support coordinator, neighbourhood officer or directly to the Customer Contact Centre at Moat, Mariner House, Galleon Boulevard, Crossways, Dartford, Kent, DA2 6QE.
